Mike Doyle

Chief Executive Officer

Through its partnership with MD Healthcare Partners, Mike serves as Chief Executive Officer of FOI. Mike is healthcare industry veteran with both clinical and executive experience. Prior to founding MD Healthcare Partners, Mike served as CEO of Surgery Partners (NASDAQ: SGRY). Under his leadership, the company grew from a single location ambulatory surgery center operation into a publicly traded national player with over 160 locations in over 30 states. Prior to co-founding Surgery Partners, Mike spent more than a decade in various leadership positions, including Senior Vice President of Operations, at HealthSouth (Encompass Health, NASDAQ: EHC). He holds a bachelor’s degree in Physiotherapy from Dalhousie University in Halifax, Nova Scotia and a Master of Business Administration from Troy State University in Alabama.

In 2019, FOI partnered with MD Healthcare Partners to provide executive management support and growth resources to expand FOI’s care delivery platform.

Charles Davis

Chief Development Officer

Through its partnership with MD Healthcare Partners, Charles serves as Chief Development Officer of FOI. Charles is a healthcare private equity veteran with more than 10 years of experience executing mergers, acquisitions, and other strategic partnerships, with a specific focus in healthcare. Prior to MD Healthcare Partners, Charles was an investment professional with HealthEdge Investment Partners, a healthcare focused private equity fund. He holds a bachelor’s degree in Business Administration from the University of Florida.

Janene Culumber

Chief Financial Officer

As Chief Financial Officer, Janene is responsible for overseeing the financial operations of FOI, its managed practices, and surgery centers. As part of the leadership team she is also responsible for ensuring the development and achievement of the organization’s strategic and financial goals. Janene has over 20 years of healthcare finance experience. Prior to joining FOI, she held the position of Senior Vice President and CFO of Moffitt Cancer Center where she worked for over 13 years. Janene also worked for KPMG, LLP for more than eight years in the audit practice serving clients whose businesses ranged from government entities and non-profit organizations to SEC clients in the industrial and consumer products industries.

Janene is a recent graduate of the University of South Florida’s MUMA College of Business where she received a Doctorate in Business Administration. Janene is also a graduate of the University of Florida where she received her Master’s in Accountancy and a Bachelor of Science in Accounting. Janene is also a Certified Public Accountant.

Lee Levanduski

Chief Operating Officer

As Chief Operating Officer, Lee is responsible for overseeing the day-to-day operations of FOI and its managed practices. As part of the leadership team he is also responsible for ensuring the development and successful implementation of the organization’s strategic and financial goals. Lee has over 30 years of healthcare experience both as a clinician and an executive-level operations leader. Prior to joining FOI, he held the position of Director with Navigant Consulting’s (NYSE: NCI) Physician Enterprise Solutions division. He also held Chief Operating Officer positions with several physician-led orthopedic practices in the State of Florida. Lee is a provider advocate and an expert in developing the necessary synergies for aligning physician practices with organizational goals.

Lee holds a Master of Science degree in physical therapy from Neumann University in Philadelphia, Pennsylvania and a Master of Business Administration from the University of West Florida in Pensacola, Florida.
