• Bring the following forms:
  1. Immunization Form and Next Year’s Athletic Packet
  2. Signed parental consent form if under 18 and not accompanied by an adult
• Get a FREE electrocardiogram (ECG) heart screening that helps identify detectable heart conditions
  • ECG Consent Form
  • ECG F.A.Q.s
• All students – all sports
• Jr. High or High School
• $10, with all proceeds back to your school
• All students under 18 must be accompanied by a parent/guardian or provide a signed parental consent form
• Please make payment at time of online registration
• Attire: T-shirt and shorts
• If you have any questions, consult your athletic trainer

All proceeds donated back to your school.
