- Jr. High or High School
- $10 *(with all proceeds donated back to your school to buy athletic training supplies)
- Attire: T-shirt and shorts
- All student athletes under 18 must be accompanied by a parent/guardian or provide a signed consent form (download here)
- Bring Immunization form and Next Year’s Athletic Packet
