Your office visit

Thank you for entrusting us with your orthopedic needs. Our dedicated team strives to deliver exceptional care throughout your visit. Keep reading to know what to expect during your upcoming appointment. 

Preparation

Please bring the following items to your appointment to ensure that our physicians will be able to see you:

  • Insurance card
  • Photo ID
  • Current medications list
  • MRI or CT report and film/CD
  • X-rays taken of your injury or condition 

Once you arrive, we will: 

  1. Make copies of your insurance card and ID
  2. Verify that all information in our database is accurate
  3. Provide you with the necessary forms for you to complete and sign
  4. Collect your copayment

For patients under the age of 18

A parent or legal guardian must accompany new patients under the age of 18 to their initial visit. The parent or legal guardian may use a Minor’s Consent for Treatment form to designate another adult to accompany an existing patient during follow-up visits. This form can be obtained during check-in.

Care & services 

You will receive results-focused treatment. Our care team is committed to delivering that experience throughout your visit. To do so, we may ask for the following: 

  • Additional filming. Even though you have provided X-rays to us, our physicians may require more information to properly diagnose or prepare you for surgery or other treatment.
  • More perspective. To gain the most comprehensive view of your condition, your physician may consult a professional from another specialty.
  • Your patience. We always do our best to meet appointment times, but our physicians may need more time than anticipated with other patients. Know that we will give the same level of personalized care to you.
  • Occasional flexibility. Our physicians are specialists who provide on-call services at many local hospitals. It is possible that your physician may be called for emergency surgery during clinic hours.

  • To cancel therapy appointments, please call your therapy office directly.
  • To cancel diagnostic tests, please call (813) 978-9700, ext. 7991, Mon-Fri, 8 AM – 5 PM.
  • To cancel surgery, please contact your care team.

No show/cancellation appointment policy

Effective June 1, 2024

Thank you for entrusting FOI with your care. When you schedule an appointment, diagnostic test, or procedure, we specifically designate the appropriate amount of time to provide you with the highest quality care. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible and no later than 24 hours before your appointment. Early notification allows our practice to better utilize appointments for other patients in need of prompt medical care. Additionally, if you need to cancel or reschedule your surgery, please contact our office as soon as possible and no later than 72 hours before your surgery.

Please see our no show/cancellation fees for the following service types:

  • Office/Therapy Appointments: $25
  • Diagnostics (includes MRI, CT, EMG, MRA): $100
  • In-office Procedures: $200
  • Surgery: $200

Fees are charged to the patient, not the insurance company, and are due at the time of the patient’s next office visit.  We understand there may be times when unforeseen emergencies occur, and you may not be able to keep your scheduled appointment. If you should experience extenuating circumstances, please contact our office as soon as possible.
