Student athletes will need to purchase a ticket below and arrive at least 15 minutes before their school’s assigned time.

• Bring the following forms:
  1. Immunization form
  2. Florida High School Athletic Preparticipation Physical Evaluation form
• All students – all sports
• Jr. High or High School
• $10, with all proceeds back to your school
• No appointment necessary
• All student athletes under 18 must be accompanied by a parent/guardian
• Make checks payable to Florida Orthopaedic Institute
• Attire: T-shirt and shorts
• If you have any questions, consult your athletic trainer

All proceeds donated back to your school.
