A professional staff of more than 500 supports our physicians, based in a 100,000 sq. ft. facility on a 12 acre campus. The main campus includes a state of the art orthopedic surgery center exclusively serving FOI patients and surgeons. With exceptional expertise in every orthopedic subspecialty, FOI is a one-stop shop, with no need to refer outside the institute.

search by anatomy: Using your mouse, roll over a specific body area to obtain more information.

search by doctor: If you know your doctor’s name, click on the letter that represents the doctor’s last name. For example, for Ellen Beatty, M.D. you would click on the letter “B.”

search by specialty: Using your mouse, click on Specialties for a complete listing of all specialties and associated doctors.

Preparing For Your Appointment

Before arriving for your appointment, please make sure that you have the following items:

  • Your insurance card
  • A photo ID
  • Any x-rays that may have been taken of your injury or condition. Our physicians may not be able to see you without your films.

Upon arrival, a patient services representative will make copies of your insurance card and ID. They will verify that all information in our database is accurate. Then they will provide all the necessary forms for you to complete and sign. Lastly they will collect your copayment.

Patients Under The Age of 18

A parent or legal guardian must accompany new patients under the age of 18 at their initial visit. The parent or legal guardian may designate someone to accompany an existing patient during follow-up visits if it is specified on a Minor's Consent for Treatment form. This form can be obtained during check-in.

Comprehensive Care & Services

Our healthcare team is committed to providing each patient with the utmost in comprehensive care and service. Nothing is more important to us than helping you get well as soon possible. We promise to apply only those treatments that we professionally feel are important to your healing process. As we care for you, please consider the following:

  • Even though you have provided x-rays to us, more filming may be necessary for our physician to attain all of the information he/she may need to properly diagnose or prepare you for surgery or other treatment.
  • At times your physician may consult a professional from another specialty.
  • We will always do our very best to meet your appointment time as close as possible. However, our physicians may need to spend a little more time with other patients than anticipated to provide the most comprehensive course of treatment. Please understand that we will give the same level of care to you.
  • Because our physicians are specialists and provide "on call" services at many local hospitals, it is possible that your physician may be called for emergency surgery during clinic hours.

For these reasons, please excuse any delays you may experience in your appointment.
