Our navigation system allows you to search by anatomy, doctor or by specialty.
search by anatomy: Using your mouse, roll over a specific body area to obtain more information.
search by doctor: If you know your doctors name, click on the letter that represents the doctors last name. For example, for Ellen Beatty, M.D. you would click on the letter B.
search by specialty: Using your mouse, click on Specialties for a complete listing of all specialties and associated doctors.
Authorizations
Authorizations for Office Visits
After your appointment is scheduled, a representative from FOI will phone your insurance company to verify eligibility, benefit type, co-payment amount, deductibles and referral requirements. This is done 3-5 days prior to your appointment. Although we make every effort to obtain authorization for your visit, many insurance companies require the patient to acquire an authorization from their primary care physician prior to the appointment. You must contact your primary care physician at least 3 days prior to your visit at FOI to ensure they have forwarded your authorization. If you arrive for your appointment without an authorization, we may be required to reschedule your appointment.
Authorization for Diagnostic Therapy Services
This department handles authorizations for all tests requested by the physicians. These include durable medical equipment, physical therapy, MRI and bone scans, among others. Each insurance company has its own rules regarding authorizing outside services. A representative from FOI will call the insurance company, the primary care physician (PCP) or the Worker's Compensation Adjuster to obtain authorization for testing or therapy. Once authorization is obtained, the representative will call the patient to schedule the test.
